Participation Waiver
Release of Liability & Media Consent — THE CENTER by Bricks 4 Kidz
Where STEAM Meets Play
A Note to Parents: Your child’s safety, joy, and confidence are our top priorities. Our programs are designed to be hands-on, engaging, and supervised by trained staff. While we take every precaution to create a safe environment, we also believe in active exploration as part of learning and development.
Assumption of Risk
I understand that participation in STEAM activities, including building, robotics, creative workshops, indoor play structures, climbing elements, slides, tunnels, and hands-on learning experiences involves inherent risks, including but not limited to minor cuts, slips, falls, bumps, collisions, and equipment-related incidents. I voluntarily assume all risks associated with participation, whether known or unknown, to the fullest extent permitted by law.
Release of Liability
I release, waive, and discharge THE CENTER by Bricks 4 Kidz, Bricks 4 Kidz Corporate, affiliates, franchisees, instructors, staff, volunteers, partner organizations, venues, schools, and collaborators from any liability arising from participation, except in cases of gross negligence or willful misconduct as required by law.
Indemnification
I agree to indemnify and hold harmless all released parties from claims arising from participation, including claims brought by or on behalf of a minor participant.
Medical Authorization
In the event of an emergency, I authorize staff to seek medical treatment for myself or my child if necessary. I understand that I am responsible for any resulting medical expenses.
Health & Allergy Acknowledgment
Parents/guardians agree to disclose any known allergies, medical conditions, accessibility needs, behavioral concerns, or other information that may affect participation. While THE CENTER by Bricks 4 Kidz will make reasonable efforts to maintain a safe environment, participation occurs at the participant's own risk.
Participant Conduct & Safety
Participants agree to follow all safety instructions, respect staff and fellow participants, use equipment appropriately, and follow posted facility rules. Failure to comply may result in removal without refund.
Indoor Play Structure & Physical Activity Acknowledgment
I understand that the facility includes indoor play structures, climbing elements, slides, tunnels, and soft-play equipment designed for active movement and exploration. Participation is voluntary, and I accept all associated risks.
Age-Appropriate Use of Equipment
Participants must use equipment and play structures only as intended and in accordance with posted age, height, or safety guidelines. THE CENTER by Bricks 4 Kidz reserves the right to restrict access to equipment when necessary for participant safety.
Personal Property
THE CENTER by Bricks 4 Kidz is not responsible for lost, stolen, or damaged personal belongings brought onto the premises.
Photo & Video Release
I grant permission for THE CENTER by Bricks 4 Kidz, its affiliates, partners, and representatives to photograph, record, and/or capture video of participants during programs and events for website, social media, marketing, promotional, and educational purposes. No compensation will be provided.
Media Opt-Out
Parents/guardians may opt out of media usage by submitting a written request to admin@thecenterbyb4k.com.
Governing Law
This agreement shall be governed by and interpreted in accordance with the laws of the State of New York.
Acknowledgment of Understanding
By electronically accepting this waiver during registration, I acknowledge that I have read, understand, and voluntarily agree to all terms contained herein.
By checking the waiver acknowledgement during booking, you confirm that you have read and agree to all terms of this waiver on behalf of yourself and/or your child.
